This unique role blends leadership with hands-on care—ensuring every decision is rooted in real experience, compassion, and a deep commitment to guiding individuals toward lasting growth, stability, and purpose.

The Case and Outreach Coordinator supports clients while building strong connections within the community. They help coordinate care, connect individuals to resources, and ensure no one falls through the cracks. Through outreach efforts and ongoing support, they create pathways for individuals to access services, stay engaged, and continue progressing toward recovery and long-term stability.
